Let’s Talk About Capsulitis

Capsulitis is a painful foot condition that can make it difficult to wear shoes, walk, or move through your day with comfort and ease. When you know what to look for, you can work with your podiatrist to ensure early diagnosis and intervention, hopefully reducing the duration and severity of symptoms.

Throughout your body, your joints are surrounded by soft tissues including tendons and ligaments. These tissues form a sort of a “capsule” around the joint and help it to function. When the ligaments in the capsule become irritated and inflamed, capsulitis occurs. This can happen anywhere in the body.

In the foot, capsulitis is most common at the joints of the big toe and the second toe. It often affects people who already have podiatric conditions such as bunions, unstable arches, and osteoarthritis. Symptoms of capsulitis include swelling and discomfort. It may also feel like there is something in the bottom of your foot or perhaps like there is a sock bunched up under your toes. Of note, studies indicate that people whose second toes are longer than their first toes are at increased risk of capsulitis, with the pain localized around the second toe.

Treatment options will include:

  • RICE (rest, ice, compression, elevation)

  • over-the-counter and prescription anti-inflammatory medication

  • taping and splinting to keep the toe in the correct position and prevent dislocation

  • physical therapy

  • custom orthotics for cushioning and support

  • surgery, but only in unusual and severe cases
